You can download this below. It also has questions for public use, should your patients ask for more information. A leaflet explaining the process has been produced, and there is also a COVID-19 update to explain the new process and how this will work for patients. This will be sent to eligible patients along with a letter inviting them for a lung health check. You can find links to both leaflets on the Useful Information section of this website.
